Formulary
Medications in the Reach Out formulary are available based upon cost, long-term treatment, single vs. multi-dose, first line is standard of care; no refills; no narcotics/controlled substances. To be eligible for these medications, you must be a Montgomery County resident, with income at or below 250 percent of the Federal Poverty level (See: Who Are Reach Out Patients?) and medically uninsured.
