About Reach Out of Montgomery County

Reach Out is a 501(c)(3) volunteer healthcare organization, founded in 1994 by three medical leaders in the Miami Valley area: Wright State University Boonshoft School of Medicine, the Combined Health District and the Montgomery County Medical Society.

Mission

Reach Out's mission is to provide access to health care services for the underserved / uninsured population in Montgomery County, without restraints, utilizing the professional skills of volunteer physicians, nurses and other health care professionals.

Eligibility

All age groups and/or disease conditions, uninsured with income at or below 250% of the poverty level. Identification is required at clinic registration; photo ID preferred, driver’s license, state ID.  Only Montgomery County residents are eligible. Patients living outside of Montgomery County will be determined on a case by case basis.

Services

Reach Out provides safety net clinics located minutes from downtown Dayton. The clinics offer an alternative to emergency visits for minor illnesses and chronic conditions that need immediate attention. Clinic visits are at no cost to the patient. Ancillary services such as lab and x-ray are limited based on current contract. Case management is available on site at Reach Out for chronic conditions such as hypertension, diabetes and asthma. Reach Out refers to Neighborhood Health Centers for continuity of care for chronically ill patients.  

Referrals

Referrals to Reach Out's sub-specialist network are dependent on availability of services requested.  In most cases the physicians’ professional fees are waived; however, patients may incur cost at the specialist office for tests or procedures performed. Payment for cost incurred is the responsibility of the patient. Reach Out performs scheduling for specialist visits.  Patients will by notified of appointment date and time by mail or phone.  Patients who do not show at scheduled appointments are not eligible to return to Reach Out for one year.

Funding

Funding for Reach Out is provided by our community in the form of foundations, grants, corporate and individual giving. A grant from the Robert Wood Johnson Foundation in 1994 initiated the Reach Out Program.
