*For jointly-provided activity applications received on or after January 1, 2015, there will be a $100 joint providership fee per application (final rates reflected above)


Effective July 1, 2014, there will be an additional $100 fee to convert credit for a live activity into an enduring activity. This fee covers assistance in using the university learning management system, verification of additional ACCME guidelines, and additional tracking requirements related to enduring materials.

Application fees are non-refundable to cover processing expenses.  In some cases, approved applications may be postponed for up to a year. Additional paperwork may be required.
