What are Community Health Workers?

Community Health Workers are individuals who, as community representatives, advocate for individuals and groups in the community by assisting them in accessing community health and supportive resources through the provision of education, role modeling, outreach, home visits and referral services.

The Community Health Worker Model empowers communities to eliminate disparities by employing professional Community Health Workers to provide a comprehensive link to community resources through family-based services focusing on success in health, education and self-sufficiency.
